Care That Reaches the Patient
Every patient enrolled in the program receives a home visit from a certified medical assistant (CMA) or registered nurse (RN), according to their needs.
Hospitals can contract with StatRCC to arrange virtual and in-home visits with StatRCC-contracted providers. This gives patients support at home while allowing clinic providers to remain in the office seeing patients.
